Anticoagulation stewardship is defined as coordinated, efficient, and sustainable system‐level initiatives designed to achieve optimal anticoagulant‐related health outcomes and minimize avoidable ADEs through (i) the application of optimal evidence‐based care; (ii) appropriate prescribing, dispensing, and administration of anticoagulants and related agents; and (iii) provision of appropriate patient monitoring and clinical responsiveness. Participants engaged with this critical support concept for patient care.

During the session, our panel of experts delved into the global landscape of anticoagulation stewardship by examining regional challenges and successes in implementing effective stewardship programmes. They provided insights into improving patient adherence, reducing adverse events, and aligning practices with the latest international guidelines and real-world evidence.
